Before going straight to these stages, it is important first to know what the glomerular filtration rate is and how it is related to the stages of kidney disease. The GFR is a test that dictates the amount of blood that pass through the glomeruli, the filters contained in the kidneys, every minute. This verifies if the kidneys of a patient are functioning well in filtering waste from his or her blood. A certain range of the GFR corresponds to a particular stage of kidney disease.

For instance, a person who has a GFR of either 90 mL/min / 1.73m² or above is in the first stage of kidney disease. Being in Stage 1 may also mean that the kidneys are functioning well, but slight damage on the kidney can be present. It may be associated with structural abnormalities or presence of protein in the urine.

A person who is in Stage 2 is presented with a GFR that falls within the range of 60 to 89 mL/min/1.73m². This means that the kidneys are more likely to have some damage and their function is mildly reduced. On the other hand, a person who has Stage 3 of the disease has a GFR that falls within the vicinity of 30 to 59. This means that the ability of the kidneys to filter waste from the blood is moderately reduced.

If the person’s GFR falls within the 15 to 29 mL/min/1.73m² range, then he or she is in Stage 4.  This stage is described as a severe reduction in the function of the kidneys. It is also in this stage that this person starts to feel unwell. Lastly, a person who has a GFR of either 14 or below is in Stage 5. The kidneys of this person is said to be in failure, or their function has lost very severely.

These stages, as mentioned earlier, aid the physician in identifying the appropriate treatment for a patient in a certain stage of kidney disease. Management for patients who fall under the first three stages of kidney disease usually focuses on monitoring of blood pressure, reducing risk factors, and observation, although the physician may already determine his or her diagnosis for a person on Stage 3.

Meanwhile, management for patients who fall under Stage 4 focuses more on planning for end-stage (or established, according to some experts) renal failure. Physicians usually prognosticate that a person who suffers from end-stage renal failure may only have weeks or months to live. On the other hand, patients who have very severe kidney disease (Stage 5) are given treatment options, such as undergoing dialysis treatment or a kidney transplant. Others may also opt to refuse undergoing dialysis anymore.

The most common and most pressing among the different kidney infection symptoms is an uncharacteristic burning sensation or pain when you pass urine. This symptom may range from simple stinging pain to an intense difficulty that will make urinating a problem for you. This type of pain is indicative of the presence of kidney injury and infection, inflammation of the kidneys, presence of stones in the organs, carcinoma of the kidneys and other related conditions such as renal failure and deprivation of blood flow in these organs. As such, intervention and management may be done as soon as the cause of the condition is identified before complications, long term damage and possibly death occur due to the progression of the condition.

The kidney pain brought by a kidney infection is accompanied by the usual symptoms of infection which are fever, chills, nausea and vomiting, general body weakness and sometimes, loss of appetite. The body, especially the flank area, becomes very sensitive to touch. A slight touch on the location of the pain will aggravate the feeling.

In order to completely relieve the pain from kidney infection, the patient should first treat the underlying cause which is the infection. Kidney stone pain may not always be present in all cases; however, it kidney stone pain is one of the most commonly reported symptoms of kidney stones. Depending on the location, there are two different types of kidney stone pain: renal colic and non-colicky renal pain.

Kidney Stone Pain

Renal colic is the term used to describe the colicky kidney stone pain that comes with the stretching of the urinary tract’s collecting system or the ureter. Kidney stone pain associated with renal colic is a result of the obstruction of the urine flow, caused by kidney stones which are 1 to 5mm in size and are able to move into the ureter.  Ureteral colic is commonly manifested by acute, sporadic, and severe pain in the flank lasting around 20 to 60 minutes. The upper outer quadrant of the abdomen may also manifest pain, depending on which part of the ureter the stone is located. Renal colic may be accompanied with nausea and vomiting. Clamminess and coolness of the skin may also be felt. Urinary urgency, restlessness, and sweating are also some of the common manifestations associated with renal colic. Obstruction of the urine flow also results to post renal azotemia and hydronephrosis.

Non-colicky kidney stone pain, meanwhile, is caused by kidney stones that distend the renal calices or the renal pelvis. The pain associated with this kidney stone location if often deep and dull. The pain can either be located in the flank or in the back, and varies in intensity—ranging from mild to severe. Many patients claim that non-colicky pain can be heightened by drinking large amounts of liquid.

Aside from pain, hematuria is also a common symptom of kidney stones. Hematuria is the presence of blood in the urine. The blood is usually caused by the scratching of the stones against the urinary tract membranes. Usually the blood appears in scanty amounts—not visible to the naked eye. But there are instances, when the blood is enough to make the urine take a pinkish tinge.

Kidney stones symptoms also include nausea and vomiting. Fever and chills can also be present.

While the adage, better safe than sorry would seem to apply, there are some factors that can give you warning signs that you should make an appointment with your doctor the very same day. If you have a fever, or if you are experiencing aches all over the body or fatigue, this could be a sign of something serious. Also, if you have a history of a urinary tract infection recently then you should get seen just to be safe.
